I, the undersigned parent or legal guardian of the below-named child, give permission for authorized leaders and volunteers of Middle Creek Baptist Church to administer basic first aid to my child as needed.

In the event of a medical emergency, I authorize church staff or volunteers to seek emergency medical treatment for my child, including contacting emergency medical services (EMS) and transportation to a medical facility if necessary.
